Maintenance Program
The ideal outcome for any of our clients is that they achieve maximum independence and are able to move out on their own into an apartment or house. Even in cases where this occurs, however, there are still needs.
The 21st Century Living Services Maintenance Program is designed to provide continued support to the client when they leave the Supported Independent Living Program and move into their own apartment. In the beginning, the client is visited on an as-needed basis, with visits decreasing over time to best meet the concerns of the client, family, and case manager. The cost of the Maintenance Program depends on the defined needs of the client as well as how quickly they are able to achieve self-sufficiency.
